Fear of Flying Assessment

This assessment is a guide only — not a psychological diagnosis. It helps you and your therapist explore possible sources of flight anxiety.

Rate each statement from 0 to 4: 0 = Not at all true, 1 = Slightly true, 2 = Somewhat true, 3 = Quite true, 4 = Very true
